The aging population continues to be a major theme in the Mississauga Halton LHIN. Over the next 10 years, the percentage of people aged 75 and older will grow by approximately 55 per cent. This growth creates increasing demands on our health care system and we are challenged to find sustainable solutions to maintain and improve health care for patients and seniors.

To ensure we are prepared to meet the needs of the Mississauga Halton LHIN’s growing and aging population now and into the future, the Mississauga Halton and Central West LHINs jointly contracted with Preyra Solutions Group to develop a Community Capacity Plan.

The final plan completed in May 2015 evaluates the level and mix of health services required by the growing and aging populations in the Mississauga Halton LHIN, including an assessment of current and future capacity and need for community-based health services for seniors. 

As outlined in Mississauga Halton LHIN’s IHSP – our three-year strategic plan – one of the key local priorities is Enhancing Community Capacity. The Community Capacity Plan is one more tool in our toolbox that will help support this priority as we continue to put a framework in place to shift care to the community, enabling seniors to continue living in their own home – where they want to be.

The Community Capacity Plan compliments Mississauga Halton LHIN’s and our health service providers’ efforts to help seniors live independently at home and provides further insights on how to create more access to care in the community. The Plan also aligns and supports Patients First, our government’s road map to strengthen home and community care as well as Bringing Care Home, the Report of the Expert Group on Home and Community Care led by Dr. Gail Donner. Our joint commitments will create a more sustainable health care system for seniors and all Ontarians. 

The Community Capacity Plan provides an opportunity for the LHIN to dig deeper into the findings to identify areas of focus and explore possible new approaches to care by understanding best practices. Results and insights gained the Plan will help inform and shape local health care system planning in the Mississauga Halton LHIN.
