Regional Program for Rehabilitative Care

The Mississauga Halton LHIN in partnership with its health service providers has undertaken a review of rehabilitation services to develop a regional strategy for Rehabilitative Care. The work includes an analysis of the current model of delivery of publicly funded rehabilitative programs and services in the Mississauga Halton LHIN including inpatient, hospital-based outpatient and publicly funded community-based rehabilitation services.

The goal is to develop recommendations for a regional rehabilitation service system model.  This model will incorporate stakeholder feedback, as well as align with best practices identified through the Provincial Rehabilitative Care Alliance work to enhance patient access to and flow through the system, improve patient experience and outcomes, and improve system integration. The recommendations will support implementation of the re-designed model of rehabilitative care within the Mississauga Halton LHIN. A Regional Rehabilitative Care Steering Committee has been created to develop the integrated regional strategy for access to and the delivery of rehabilitative care services.

Rehabilitative Care Alliance

The Rehabilitative Care Alliance is a province-wide collaborative established in April 2013 to develop recommendations that lead to improved long-term clinical outcomes for Ontarians and increase community capacity so that people have access to rehabilitative care when and where they need it.

Rehabilitative Care Portal

Rehabilitative Care in Ontario ( provides general information about rehab and connects users to LHIN-specific pages where they can search for services in their area. Providers can use it to find rehabilitative care services for their patients; the public can use it to locate rehabilitative services. The Rehabilitative Care Alliance is funded by and reports to Ontario’s 14 Local Health Integration Network (LHIN) CEOs under the following governance structure:

The Rehabilitative Care Alliance is pleased to announce that it has received approval from Ontario’s 14 LHIN CEOs for another two-year mandate to run from April 1, 2017 to March 31, 2019. This will be the Rehabilitative Care Alliance’s third mandate. The Alliance will continue to make recommendations to support enhanced outcomes and increased community capacity for rehabilitative care by improving:

  • Value 
  • Equity in the delivery of rehabilitative services across the care continuum

Implementation of recommendations is the responsibility of LHINs and other health system partners.
