Apply now for the 2018 Minister’s Medal

The Ministry of Health and Long-Term Care (ministry) has announced the launch of the sixth annual Minister’s Medal Honouring Excellence in Health Quality and Safety. This annual recognition program aims to honour excellent work by health care providers across the province. This award is an opportunity to recognize system champions that partner with patients and have demonstrated exceptional work in collaboration and achieved sustainable results, while promoting system value and quality in the health care system.

 

The theme of the 2018 Minister’s Medal is Innovating Integration, which recognizes the creative and innovative initiatives being undertaken across the province to foster an integrated health system in order to achieve better patient and provider experience, better health outcomes and better value in health care delivery. This theme also recognizes health champions focused on strengthening partnerships and integration to ensure a seamless patient journey.

 

The ministry will be awarding medals in three categories: one for teams working together to provide excellence in high-quality care; one for individual providers who have demonstrated dedication, vision and leadership in championing high quality care; and, one that recognizes the important contributions of patients, families, and caregivers who have undertaken innovative initiatives within their communities.

Application deadline

Application templates are now available for download from the Minister’s Medal webpage at http://www.health.gov.on.ca/en/pro/programs/transformation/minister_medal.aspx.

Applications are due to your Local Health Integration Network office via email by 5 p.m. on August 24, 2018.

Winners will be officially recognized at Health Quality Transformation 2018, an annual event hosted by Health Quality Ontario on October 17, 2018. More information on the program, including profiles of previous years’ winners and honourees, is available on the Minister’s Medal webpage

Applicants will be notified once the individual and team-based winners have been selected.

2017 Minister’s Medal Honour Roll Recipients

We are extremely proud to say that we were selected as Honour Roll Members for demonstrating outstanding performance across the selection criteria. 

Hanaa Elkalza, Regional Patient and Caregiver Engagement Coordinator, was recognized for her passion and enthusiasm for engaging patients in building a system of care that is conducive to their needs.

The Mississauga Halton LHIN was also recognized for the Care Coordination Program of Work for the re-design of our care coordination approach to enable delivery of better integrated consistent, patient-centred care at the neighbourhood level.

Caregiver Respite Program: 2016 Minister’s Medal Winner – Team Recipient

The Caregiver Respite Program in the Mississauga Halton LHIN was recognized with the 2016 Minister's Medal for improving services and supports that enable caregivers to care for their loved ones and also take care of themselves.
