Minister’s Medal

The Minister's Medal Honouring Excellence in Health Quality and Safety is a competitive, annual award program that recognizes excellent work done by health care partners across the province. Award recipients have demonstrated excellence in engaging patients as partners, as well as enhancing value and quality in our health care system.

The theme of this year's awards is Innovating Integration, which focuses on recognizing the innovative and transformative initiatives being undertaken across the province to improve patient and provider experiences, support better value, and improve health outcomes across the province.

Putting Patients at the Heart (PPATH): A Seamless Journey for Cardiac Surgery Patients: 2018 Minister's Medal Winner - Team Recipient

Trillium Health Partners and SE Health was recognized with the 2018 Minister's Medal for their innovative cardiac program known as Putting Patients at the Heart (PPATH). PPATH redesigned the care journey for cardiac surgery patients by streamlining and simplifying access to follow-up care so that cardiac patients return home sooner and with better supports.

2017 Minister’s Medal Honour Roll Recipients

We are extremely proud to say that we were selected as Honour Roll Members for demonstrating outstanding performance across the selection criteria. 

Hanaa Elkalza, Regional Patient and Caregiver Engagement Coordinator, was recognized for her passion and enthusiasm for engaging patients in building a system of care that is conducive to their needs.

The Mississauga Halton LHIN was also recognized for the Care Coordination Program of Work for the re-design of our care coordination approach to enable delivery of better integrated consistent, patient-centred care at the neighbourhood level.

Caregiver Respite Program: 2016 Minister’s Medal Winner – Team Recipient

The Caregiver Respite Program in the Mississauga Halton LHIN was recognized with the 2016 Minister's Medal for improving services and supports that enable caregivers to care for their loved ones and also take care of themselves.
