Professor at the Institute of Health Policy Management and Evaluation at U of T and Research Chair in Implementation and Evaluation Science at THP’s Institute for Better Health. Dr. Wodchis joins this year’s Primary Care Clinic Day as the keynote speaker to discuss the health system imperative to implement Patient-Centred Medical Homes and integrated approaches to care. Dr. Wodchis looks at the key factors that give rise to the enhanced need for PCMH (i.e. Challenges of multi-morbidity and patients with complex health needs) and how to manage such patients in the community. Dr. Wodchis shines light on the barriers of Ontario’s health care system and presents innovative ideas toward health system transformation and striving to deliver better, more integrated patient care.
